You May Qualify for Additional Services
If you are currently enrolled in Medicaid, you may be eligible for enhanced services and support through New York State’s Social Care Network (SCN). These additional services are designed to reduce barriers, improve your quality of life, and support long-term health and recovery.
As a participating SCN provider, we offer free Health-Related Social Needs (HRSN) screenings to help connect you to the resources you may qualify for.

What Is an HRSN Screening?
An HRSN screening is a quick, confidential check-in that helps us better understand the day-to-day factors affecting your health and stability.
With your permission, we may ask about:
Housing and utilities
Food access
Transportation
Employment and education
Interpersonal safety
Financial or social stress
Your answers help us identify what supports and services may be available to you through New York State’s Social Care Network, all focused on helping you live a healthier, more secure life.
